The key phrase of the question is: "to at least 10 GB"
That means it starts there and can grow more. With the other two options (answers A and D) are maximums, and won't grow any larger than 10GB.
By the way, this is a horrible question, because Technet shows the FileSize regkey as a valid way to increase the cache, but they also produce the tool that does it. The tool is for scripting, which is why I originally thought it wasn't the answer, because you're only changing ONE computer. However, I think its to trick you, because of the "to at least 10GB" which means a minimum.

After research, I'm thinking the ports are still 443 on the Gateway and 3389 on the Session Host. The reason being the Session Host doesn't do 443 whatsoever. The Gateway converts the 443 into 3389, and then talks to the Session Host via 3389. I also read this:

Gateway listens on 443, and Session Host listens on 3389. Seems simple, so that's why I'm going to specify answers A and D.

I agree with all yours, except for the RDP or HTTPS answer, where you are giving a company a preconfigured RDP file. I think the answer is HTTPS. Whenever you specify Connect From Anywhere settings, you are turning on HTTPS. I had a trainer verify this, and a pretty smart systems engineer say the same thing. I just found this as well: "By using this gateway, you not only provide pre-authorized access to you terminal server, you also gain the benefit of using RPC over HTTPS, eliminating the need for additional ports to be open on you firewall and the ability to use RDP behind most corporate firewalls using port 443."
At first I didn't agree with your opening ports 3389 and 443 on the Session Host server. I did some research, and I was still thinking it's 443 on the Gateway, but I'm thinking you're probably right. So, when I take the test (again...) I'm going to put 443 and 3389 on the SESSION HOST servers.
I spent HOURS research that AppVCache tool question, and you are exactly right: It's B!
